2014: Recent Amendment
to Several Probate Forms:
The following Probate Forms have been amended by the Ohio Supreme Court (discard prior versions): Adoption 19.2 & 19.3, Certificate of Transfer 12.0 & 12.1, Disinterment 25.0-25.6
When using AREIS, make sure you are using the most updated information and attach this to the inventory.

|Please note that effective January 1, 2013, new probate forms for Application for Name Change, Adult and Minor, (21.0 and 21.2) and Application for Appointment of Guardian of Alleged Incompetent (17.0) must be used. Please disregard all previous versions of these forms.|
|Effective January 1, 2012, the cost of Birth Parent Assessments, in the amount of $350.00 will be added to the deposit for Relative, Independent and Private Agency Adoption cases, if applicable. Please contact the Adoption Department with questions at 419-213-4794|
Effective August 1, 2012
the Court will require FBI Record Checks on ALL new Adoption
cases except Adult Adoptions.
These checks are in addition to BCI record checks and will be performed by the Court, with costs to be paid by the Petitioners at the time of the Record Check.
Cost for both record checks is currently $46.00

Form 24.6 - Judgment Entry of Insolvency
Requirement For All Partial Accountings
Effective January 1, 2012, All partial accountings filed with the Court will require a Bank Certification to verify assets remaining for all bank accounts.
