Effective January 1, 2012, the cost of Birth Parent Assessments, in the amount of $350.00 will be added to the deposit for Relative, Independent and Private Agency Adoption cases, if applicable. Please contact the Adoption Department with questions at 419-213-4794
Effective August 1, 2012 the Court will require FBI Record Checks on ALL new Adoption cases except Adult Adoptions.
These checks are in addition to BCI record checks and will be performed by the Court, with costs to be paid by the Petitioners at the time of the Record Check.
Cost for both record checks is currently $46.00
Please note that Effective January 1, 2012  New standard probate forms for Representation of Insolvency must be used. Please disregard all previous versions of Insolvency forms

Bank Certification Requirement For All Partial Accountings
Effective January 1, 2012, All partial accountings filed with the Court will require a Bank Certification to verify assets remaining for all bank accounts.
