face-to-face home visit with the adoptive parents and the child no
later than seven days following the adoptive placement. |

Monthly
face-to-face home visits with the adoptive parents and the child
from placement date until finalization date. (Approximately 6-8 home
visits) |

Two
reports due to the court. One before the interlocutory order and one
(JFS01699) due no later than twenty days prior to the final hearing
date. |

Supervision
home visit fee (due at time of visit) is $110.00. |
