Standard Forms
This column represents forms that were adopted by the Ohio Supreme Court and or the Lucas County Probate Court for official use. These forms can be printed and used accordingly.

Web Interactive Forms
This column represents forms which were taken from the Standard Forms column, then modified to allow for web interaction. These forms can be completed on-line and then printed.

Please note that if there is not a link associated with a particular form or form packet, (form name or form packet description will be light blue and will be underlined), then the form  or the form packet is not currently available but is under development

Packets of forms are also available and are denoted by the shaded boxes below. Form packets include all of the required forms to file that particular packet. You may be required to file additional forms in conjunction with the forms within a given packet depending on the various circumstances which may be involved.
