for obtaining a certified copy of a Birth Record or a Death Record
is $5.00 for each copy
requested.
If
submitting your request through the mail, please send money orders
only (no checks and please do not send cash).
Please
send your request and a money order in the amount of $ 5.00 for each
copy requested, along with a self addressed stamped envelope, to the
